What the human evidence actually is

Phase 2 established weight reduction as monotherapy. The REDEFINE phase 3 programme tested it combined with semaglutide as CagriSema, with REDEFINE-1 reporting roughly 23% mean weight reduction. Novo Nordisk filed its FDA application on 18 December 2025 with a decision anticipated in late 2026 — no PDUFA date has been made public. Worth knowing: REDEFINE-4, the head-to-head against tirzepatide, missed its endpoint. As of August 2026 CagriSema is not approved.

How it is supposed to work

Mimics amylin, a beta-cell hormone that signals satiety through the area postrema — a pathway separate from GLP-1, which is the rationale for combining them.

Safety

Nausea and vomiting, injection site reactions. Long-term safety not established.

Storage and handling

Lyophilised peptide is generally stored at 2–8 °C, or below −20 °C for long-term storage. Reconstituted in bacteriostatic water (0.9% benzyl alcohol) a solution is typically treated as usable for about 28 days refrigerated, versus roughly 24 hours for plain sterile water, which has no preservative. Add the water slowly down the vial wall and swirl — never shake, which shears peptide chains and foams the solution.
